What is transition?
Transition
describes the process by which a student moves from school to adult life in the
community. Transition services focus
on the skills a student needs to reach postsecondary goals in
education/training, employment and independent living skills (if needed).
Transition includes age appropriate assessments, courses of study,
activities, and annual goals that will reasonably enable a student to attain
postsecondary goals. In
North Carolina
, an initial transition component is developed as part of the student’s IEP
upon turning 14 years of age with a focus on the student’s needs, preferences,
interests and course(s) of study.
Who should be involved in Transition planning?
- Students
- Parents
- School personnel
- Adult service providers (with
written consent of parent or student, if 18 years or older)
What are the benefits of transition planning?
- To ensure that linkages with
adult services are created to support students who need them after leaving
high school
- To ensure individuals are
employed in supportive or competitive employment or engaged in volunteer
work, an adult day program, training program, postsecondary education, or
whatever is necessary to provide a meaningful life after high school
- To help fulfill the dreams and
visions students have about who they are, what they want in life, and where
and how to achieve their goals
Transition Program for Young Adults (TPYA)
New
Hanover
County
School
’s Special Education and Related Services also offers the Transition Program
for Young Adults as an option for students with moderate to severe disabilities
who are 18 and older and have had a minimum of four years in a traditional high
school. TPYA has received
national recognition as an “exemplar school transition program.”
The Transition Program for Young Adults (TPYA) is a community-based
program with three community sites. This
program, implemented since 1997, focuses on exploring the many facets of living
and working in our local community. TPYA
provides the opportunity for students to learn, maintain and generalize skills
to the real world and to assist them in achieving their personal maximum level
of independence as they transition from school to adult life. Because
of TPYA’s community-based focus, students have the opportunity to practices
many skills daily such as paying bills, making a bank deposit, getting a
haircut, purchasing groceries, etc., in settings where they would naturally
occur. Students also have extensive
opportunities to practice decision-making, problem-solving, goal setting,
personal choice-making and self-advocacy skills.
